You just upload it. I printed mine out, got the signatures, took a documented picture In notes, and than uploaded. Idk if that’ll work for you or not, but I’d recommend it as it’s easier. I think you can submit a picture. To be safe, I would ask or scan the page as a pdf. You can scan using a printer or using Google Drive.
